Student Field Trip Registration

This form must be completed by faculty or staff who are planning to conduct a student field trip away from the University's campuses. In addition to this form, a roster naming all trip participants must be kept on file in the sponsoring department's office for a period of two years. This ensures that all participants are covered under the University's Student Field Trip insurance policy.

Please submit this form prior to departure and be sure to keep a complete roster of the trip participants on file in your office.
