Mandatory Health & Safety Training

Prior to coming to campus, you will need to watch the training video, complete the assessment, and report your vaccination status. Please use the links below for each step.

  3. You will report your vaccination status in Medicat Public Health Portal through SSO (If you have already submitted your information previously, you do not need to submit it again through Medicat.) If you have not received your loyno email address yet and are unable to submit your status information through the portal, please email it to covid@loyno.edu.

You will need to login with the following credentials. Clicking the link will prompt you to login to Medicat using your Loyola Microsoft account.

  • Your username is your full Loyola email address.  xxxxxxxx@loyno.edu  
  • Your password is your Microsoft password.

If you have changed your Microsoft password, use what you changed it to.  If you have not yet changed your Microsoft password, the default password is the first two letters of your first name, followed by the last 4 digits of your social security number, then an uppercase LU.  ex. abXXXXLU 

If you cannot sign in, go to office.com to reset your Microsoft password. If you need assistance, from your Loyola email address, email support@loyno.edu

Vaccination Status Reporting FAQ's:

  • Am I required to share my vaccination status? Yes, per university policy, you are required to report your vaccination status.
  • Will my information be shared with anyone? Yes, your direct supervisor will know your vaccination status.
  • Do I need to respond if I primarily work remotely? Everyone needs to respond. You will need to log in to your Public Health Portal and under the heading/tab FORMS, complete the Online Only Attestation Form. 
  • Instructions for Vaccinated:
  1. You will need to take 2 steps: 
  2. Step 1 Enter COVID-19 vaccine info - Enter the date(s) you received your COVID-19 vaccination under the heading/tab IMMUNIZATIONS.
  3. Step 2 Submit proof of COVID-19 vaccine - Upload proof of your COVID-19 vaccination under the heading/tab UPLOAD. You can submit a copy of your COVID-19 vaccination card or LA Wallet or any record that is legible and clearly displays the vaccine name and date(s) received.
  • Instructions for Unvaccinated:
  1. You will need to log in to your Public Health Portal and under the heading/tab FORMS, complete the Unvaccinated Employee Form.
  • How can I get assistance with completing the portal forms? You can schedule a 1:1 virtual appointment with a member of the Public Health Team by clicking here, OR You can stop by the Student Affairs suite, Danna Center 205 (directly above Starbucks) Monday-Friday, 8:30 a.m. to 4:45 p.m. for a walk-in, in-person appointment with a member of the Public Health Team.
  • How will this information be used? Your COVID-19 vaccination information will be used to protect our community in ways such as being able to waive or reduce testing and quarantine requirements, making public health informed decisions for campus, and engaging in administrative planning.
